Dilated perivascular spaces, cognitive decline and incident dementia


  • Severe periventricular space (PVS) pathology is a marker for increased cognitive decline and dementia risk, independent of other small vessel disease (SVD) markers.

Why this matters ?

  • PVS are seen in the basal ganglia (BG) and centrum semiovale (CSO) in older adults, and may be associated with discrete pathophysiology, and potentially, impairments in different cognitive domains.

  • PVS pathology may constitute a potentially important biomarker for early dementia diagnosis, subtyping and prognosis, and may have a significant impact on individualized treatment; as such, further research of PVS etiology and sequelae is required.
